We CAN NOT sign the student's temps application "DMV MV3001" form until students are very close
to 15 1/2.
Our signature is only valid for 60 days so if your form was signed more than 60 days ago you will need to get a
new form. You MUST have a BTW scheduled BEFORE you can get your form signed. We need to know the
date of your 1st lesson.
